Telling stories is one of the best ways to explain partnership and show others what it looks like. By telling your team's story you can help spread successful practices throughout Kaiser Permanente.

Use this tool to create a short animation that tells the story of your unit-based team project. You will be able to share it with others when you are done.

Your story can be told in four simple parts:

  • Once upon a time: The introduction. Set the stage; where does your story start?
  • Suddenly: The story’s characters are confronted with a challenge.
  • And then: The characters take action to address it.
  • Happily ever after: How did it all work out?

It’s best to gather all the information you will need before you begin your story, so gather up your team binder and other documents. Complete the printable (DOCX) version of the online form by yourself or with your team--you may want to ask your UBT consultant for assistance--and then enter it online. NOTE: Once you start filling out the online form, you have to work through to completion; you cannot save it when you're partway through and come back to it later.
