Description: At the beginning of 2010, Bernard Tyson, executive vice president of Health Plan and Hospital Operations, and the regional presidents came up with a series of actions and effective practices to prevent injuries. One of the actions is to report Workplace Safety Never Events.

The purpose of the Never Event reporting is to inform higher-ups so serious injuries are investigated quickly. That way, we can determine the cause of the injuries and put mechanisms in place to prevent similar injuries from occurring.

This tool provides a list of workplace safety incidents that will be treated as Never Events. Reporting requirements also are included.
