Colorado
Serves 495,497 members in 34 medical offices. Of its 268 unit-based teams, 192 (72 percent) were rated high performing as of December 2025.
Georgia
Serves 334,408 members in 27 medical offices. Of its 97 unit-based teams, 72 (74 percent) were rated high performing as of December 2025.
Mid-Atlantic States
Serves 751,651 members in Maryland, Virginia and the District of Columbia in 41 medical offices. Of its 254 unit-based teams, 177 (70 percent) were rated high performing as of December 2025.
Northern California
Serves 4.6 million members in 203 medical offices and 21 hospitals. Of its 1,366 unit-based teams, 500 (37 percent) were rated high performing as of December 2025.

Northwest
Serves 612,290 members in Oregon and Southwest Washington, in 52 medical and dental offices and 2 hospitals. Of its 380 unit-based teams, 279 (73 percent) were rated high performing as of December 2025.

Southern California and Hawaii
Southern California serves 4.9 million members in 200 medical offices and 16 hospitals. Hawaii serves 271,667 members throughout the islands, in 20 medical offices and 1 hospital. Of 1,295 unit-based teams, 1,076 (83 percent) were rated high performing as of December 2025.
Washington
Serves 561,980 members in the Puget Sound area and east to Spokane with 32 medical facilities. It has 211 unit-based teams as of December 2025.
